Coordination

At Tunza Care Services, our Support Coordination team helps NDIS participants make the most of their plans with confidence and clarity. We understand that navigating the NDIS can be challenging, so our coordinators are here to guide you through every step — from understanding your plan to connecting with trusted service providers. Our goal is to help you make informed decisions, exercise choice and control, and build lasting independence.

There are three levels of Support Coordination under the NDIS — each designed to meet different participant needs depending on the level of complexity and support required.

Overview

  • Assist participants to understand and implement their NDIS plan.
  • Connect participants with appropriate service providers and mainstream supports.
  • Ensure participants make informed decisions and exercise choice and control.
  • Monitor service delivery and resolve issues with providers.
  • Advocate for participants where needed to achieve goals in their plan.
  • Support participants to build capacity and independence over time.

Level 1

Support Connection

 

  • Helps participants find and connect with service providers.
  •  Provides basic guidance on using NDIS funding effectively.
  • Offers short-term assistance for setting up services.
  • Focuses on practical, day-to-day coordination.

Ideal for: Participants who are generally independent but need some initial help linking with supports and setting up their plan.

Level 2

Support Coordination

 

  • Works with participants to understand complex supports and services.
  • Coordinates multiple providers to ensure seamless support delivery.
  • Assists with crisis resolution, conflict management, and problem-solving./li>
  • Helps participants navigate NDIS processes, including plan reviews.
  • Provides guidance on building skills for independence over time.

Ideal for: Participants with moderate complexity who need regular coordination, guidance, and problem-solving support.

Level 3

Specialist Support Coordination

 

  • Supports participants with high complexity or significant barriers to access.
  • Works with participants with multiple, complex needs or risk factors.
  • Advocates for participants in difficult or high-risk situations.
  • Liaises with external systems such as healthcare, justice, housing, and disability services.
  • Provides intensive guidance for decision making and capacity building.

Ideal for: Participants with highly complex circumstances who require specialist knowledge and collaboration across multiple service systems.
